Please
remember to bring your insurance card with you. If you are
a member of an insurance plan that has managed care, you
will need to have a referral from your Primary Care Physician
to see your allergy specialist. Be sure to have your Primary Care
Physician call in the patient's referral to the insurance company
so that your visit will be covered. If you do not have a current
referral, you may be financially liable for all medical services.
Please remember that each employer who
purchased insurance for employees may have specific benefits. Not all policies cover
allergy services, so please check with your company benefits representative before you
come for your visit.
If you have reached the annual
deductible on your insurance plan, and have allergy benefits, our office will submit
all treatment to your insurance company. If you have not met your insurance deductible,
then you will be responsible for paying the doctor until the deductible has been satisfied.
We expect payment at the time of services, unless you are on a budget plan arranged for you
by the office manager. Our staff will work with you to help with financial and/or
insurance concerns.
Please keep
us informed of any changes in your name, address, phone number
or insurance as it may be needed for emergencies.
